Therapy Agreement

By Sabina Nazarova

1. Introduction

The following is an agreement between you, the client, and me, your therapist, for therapeutic work. It provides an outline of how I work so that you are fully informed. Please take some time to read through it and please raise with me any concerns you have about any aspect of this Therapy Agreement.

2. Scope of Services

The Therapist agrees to provide professional therapy services to the Client. The specific goals and treatment plans will be collaboratively established during the initial sessions and may be adjusted as therapy progresses.

3. Assessment

The therapy process will begin with an assessment phase where we will explore your current concerns, personal history, and therapy goals. This assessment will help guide our therapeutic approach and establish a framework for our work together.

4. Therapeutic Goals

It is important to establish therapeutic goals to guide our work together. These goals will be based on your identified needs and aspirations. We will regularly review and revise these goals to ensure they remain relevant and achievable.

5. Confidentiality

All information shared within therapy sessions is confidential.

6. Session Structure

  • Duration: Individual therapy session will last approximately 60 minutes. Couple Therapy session will last 90 minutes.
  • Frequency: Sessions will be held weekly, as agreed upon by both the Client and Therapist.

7. Fees and Payment

  • Payment Method: Payment for each session must be made online through our website before the session is accepted. Upon receipt of payment, your session will be confirmed.

8. Consistency in Session Scheduling

Consistency in session scheduling is vital for the effectiveness of therapy. We will work together to establish a consistent time slot for your therapy sessions, which will be reserved for you each week. Please make every effort to attend your scheduled sessions punctually.

9. Concluding Therapy

Therapy progresses towards the achievement of agreed-upon goals. As goals are met, therapy naturally concludes. Typically, therapists gradually decrease session frequency as clients demonstrate progress and increased self-reliance.

Open dialogue is encouraged throughout the therapeutic journey. You feel therapy has served its purpose, we will discuss the best approach for concluding sessions. A gradual decrease in session frequency may be recommended to ensure a smooth transition.

10. Termination of Therapy

The Client or Therapist may terminate therapy at any time. If the Client wishes to terminate therapy, it is recommended to discuss this decision during a session to ensure proper closure and to discuss any further recommendations.
